Organizational and Service Opportunity Update

Thank you for your partnership with California State University, Long Beach (CSULB). We appreciate our ongoing partnership with your organization and want to make sure we have the most up-to-date information about the off-campus learning opportunities you’d like to make available to students. This form allows you to:

  1. Review and update basic Organizational and Main Contact Information
  2. Review and update Site Requirements and Safety
  3. Update or submit new Student Service Opportunities

Ideally, a representative of the organization who is familiar with the learning activities that university students will be engaged in, as well as your safety policies and procedures, should complete this form. If you have multiple service opportunities, you may submit this form multiple times.
