Organizations that have already completed and received partnership approval may use this form to make learning opportunities available to CSUMB students.

You will need the following information to create opportunities:

• Site contact information
• CSUMB program with which to associate the opportunity (e.g. Service Learning, CHHS)
• Opportunity details and requirements
• Safety and risk information

It may take about 10 minutes to complete.
