Dialysis Social Work InternAt — DaVita Dialysis
In this position you will have the opportunity to practice medical social work in a dialysis facility where you would gain experience in biopsychosocial assessments, risk assessments, patient care coordination and case management, supportive counseling experience, grief and loss experience, mandated reporting and community partnership with hospitals, skilled nursing facilities, discharge planners, and local transplant centers. You would also practice patient advocacy on micro and mezzo levels and work closely with an interdisciplinary team of registered nurses, dietitians, and nephrologists to support patient care, quality of life, and clinical outcomes.
Psychosocial assessments, creating care plan goals, risk assessments, mandated reporting, how to work with various mental health diagnoses in a healthcare setting, supporting counseling skills, how grief and loss presents in pts living with chronic illness, how to collaborate with different disciplines on common treatment goals, patient advocacy in a healthcare setting and more.
DaVita provides formal and experiential training on assessments, mandated reporting, case conceptualization, documentation, transplant referrals, as well as opportunities to shadow MSWs/LCSWs on-site. Additionally, interns complete a series of online trainings orienting them to DaVita.
