Opportunity Summary 

The MSSP program is an intensive care management program that serves medically frail older adults, 65 + years of age in their homes in order for them to stay at home safer and longer with various services and supports. Participants are overseen by a staff of social workers and nurse on a monthly and as needed basis. Staff of the MSSP program provide advocacy, case management and support to participants.

Opportunity Training 

Students will be shadowing the current social worker staff and nurse in order to learn all that the staff of MSSP does and what they the student will be responsible for. There will be oversight and many hands on opportunities for students to learn how to manage a caseload of medically frail older adults.

Additional risk specific to this opportunity 

Stanislaus County policies and procedures will be reviewed by the student. There is an online Driver Safety course that the student will need to complete as well.
