Case Management InternshipAt — Tracy Community Connections Center
We are a 501(c)3 nonprofit whose mission is to empower individuals to rebuild their lives through comprehensive support, resources, and community collaboration. We provide recuperative care, housing navigation, full-time case management, a mobile shower program, and a street outreach program while partnering with the city of Tracy, local agencies, hospitals, churches, and community organizations to care for the unhoused in Tracy. As a case management intern, you would have the opportunity to learn how to provide resources for our clients and help us build organizational capacity to do meaningful and rewarding work.
One-on-one training with our Director of Case Management to learn our case management software, HMIS, and client intake processes. Collaboration with other case managers to learn how to best advocate for clients and find them resources.
Some of our clients have mental health challenges and can be unpredictable.
