COVID-19 Student Waiver

Academic Internship/Program Site Covid-19 Acknowledgement

Should I engage in-person for any part of the experiential learning activity to which this placement pertains, I understand, acknowledge and agree with each of the following statements:

• My participation in an on-site experiential learning activity during the semester selected below is voluntary and not required by the University.
• Alternatives are available to an on-site experiential learning activity, and I will not be penalized if I choose to participate in an alternative activity rather than an on-site experiential learning activity.
• If at any time I determine that the risks of proceeding with my on-site experiential learning activity are too great, I may request an alternative assignment and that I will not be penalized for making such a request.
• The risk of the transmission of COVID-19 increases whenever work is done where contact with others is inevitable simply because of the close proximity and length of person-to-person interactions.
• My on-site experiential learning activity may not test or have plans to test any of its employees or clients for COVID-19. Therefore, it is possible that one or more of my clients, fellow employees or other people with whom I come into contact at my experiential learning activity site are capable of transmitting COVID-19 to me or others, even if they themselves remain asymptomatic.
• The risks of transmission of COVID-19 during my on-site experiential learning activity cannot be eliminated, but may be mitigated by practicing good prevention behaviors such as wearing a face covering whenever contact with another individual is possible, frequently washing my hands with soap and water, refraining from touching my face before sanitizing my hands, and practicing sufficient physical distancing.
• Notwithstanding any practices, measures or methods employed or adopted by my on-site experiential learning activity, there will still be a risk and possibility of contracting or transmitting COVID-19 during my experiential learning activity.
• While I am present at my on-site experiential learning activity, I will follow all health & safety measures prescribed by the experiential learning activity site.
• If I voluntarily choose to participate in on-site experiential learning activities, I will receive no additional internship credit or benefit from the University for any additional hours worked beyond those required to complete the experiential learning activity.
• If the University determines that all on-site experiential learning activities must end and be completed remotely, I will end my on-site experiential learning activity.
• I will promptly report to the University any changes in circumstances at my experiential learning activity site, including the site’s decision to change my internship from on-site to virtual and vice versa.
• If I contract COVID-19 and/or am exposed to COVID-19 during my on-site experiential learning activity , I will report that to the University.

After considering the information above, I understand and acknowledge that, should I engage on-site experiential learning activities during the semester selected below including, I do so in the knowledge that alternative assignments are available and that I will suffer no penalty if I choose to not participate in on-site experiential learning activities. My signature below acknowledges that I have read, understand and will abide by each of the statements included in this document regardless of whether I intended to engage in an on-site experiential learning activities at this time or subsequently have the opportunity to do so.
