Faculty should use this form to set up their service activities each semester. After entering the information below and submitting this form, an email will be sent to the partner site identified requesting that they confirm the service opportunity description provide any additional descriptive information they wish to along with risk management information need for approval.

Important:

Service that requires students to be alone with vulnerable populations (minors, people living with mental illnesses including older adults). Positions requiring this will not be approved.

Where service duties require students to drive, the site must either determine whether the student has appropriate coverage for themselves, or provide motor vehicle insurance to the student to cover them while conducting driving duties.
