Position: Case Manager Intern
Position Summary: The mission of Family Promise of Orange County is to help families experiencing homelessness achieve housing stability.
Case Manager Intern shares accountability for the coordination of supports and services and long-term planning of multiple complicated cases.
- Answer all incoming phone calls; complete contact sheet; send emails and referrals; complete assessments; complete triages.
- Conduct intakes, background checks and drug screenings with potential families being admitted into the program.
- Communicates with the Program Manager and Executive Director as needed to ensure family’s well-being and ability to perform designed program goals. This will include discussing cases with the Program Manager to ensure that client is completing program requirements.
- Conduct weekly case management while using the integration of social services/case management functions into the client care, and planning processes with other external service organizations, agencies and healthcare facilities.
- Create and establish family and case management goals for each client, while supporting these families in obtaining these goals.
- Follow and track client progress to assure they are meeting their goals and records progress appropriately and professionally.
- Mobilizes resources and interviews, as needed, to achieve expected goals within the desired time frame.
- Ensure that client individual case plans are appropriate and necessary and are carried out within the established time frame.
- Participates in the development and implementation of client care policies and protocols in order to provide advice and guidance in handling special cases or client needs.
- Serves as a client advocate as appropriate and necessary.
- Preform occasional Transitional House visits inspecting if the house is in a clean and livable state.
- Move families in and out of Transitional Houses.
- Performs exit interviews/Surveys and follow up procedures for clients who have graduated the FPOC programs.
- Reports success stories, progress and statistics regularly and on request.

- **WEEKLY SUPERVISION Once a Week. It is mandatory.**
•Update client case files, with documentation provided from the client
•Connect clients with resources and and perform linkage
•Shadow case manager when conducting case management, intakes, and interviews
•Answer phone calls, conduct phone interviews/assessment, conduct triage, and send emails
Training:
•Initial Internship Orientation & Goal-Setting Session
•Weekly one-on-one supervisor meetings
Learning Outcomes:
•Increase verbalization with a specific client population, learn computer and assessment skills, and how to write clearly and concisely.
•Work within the agency's policies and procedures, increase verbalization with coworkers, how to take direction, and gain knowledge regarding client population.
•Take initiative, identify dangerous/crisis situations, and how to find community resources.
