Opportunity Summary 

The Case Management Intern will assist in providing comprehensive support services to clients within a social services agency. Duties include conducting client assessments, developing service plans, coordinating resources, advocating for client needs, and maintaining accurate documentation.

Tasks:

\n

Conduct client intake interviews to gather demographic information, assess needs, and identify goals.

\n

Collaborate with clients to develop comprehensive service plans tailored to their unique circumstances and preferences.

\n

Coordinate referrals to external agencies and service providers to address housing, employment, healthcare, and other needs.

\n

Advocate on behalf of clients to ensure access to entitlements, benefits, and community resources.

\n

Maintain accurate and up-to-date client records, progress notes, and documentation in electronic databases.

\n

Participate in interdisciplinary team meetings to review cases, discuss strategies, and coordinate services.

\n

Provide crisis intervention and support to clients experiencing emergencies or urgent situations.

\n

Conduct follow-up assessments and evaluations to monitor progress, identify barriers, and adjust service plans as needed.

\n

Assist with program evaluation, data collection, and reporting activities to track outcomes and measure impact.

\n

Engage in ongoing professional development activities, including trainings, workshops, and seminars related to case management practice.

\n

Training:

\n

General employer orientation: An initial orientation session to familiarize interns with agency policies, procedures, and expectations.
Weekly scheduled one-on-one mentor meetings: Regular meetings with a designated mentor to provide guidance, feedback, and support throughout the internship.
Job shadowing: Opportunities to observe experienced case managers in action to gain insights into best practices and strategies.
Workshops/skills training: Participation in workshops or training sessions focused on relevant topics such as cultural competency, crisis intervention, and trauma-informed care.
Provision of work samples: Access to sample documents, case studies, and resources to facilitate learning and skill development.
Overview/contextualization of assigned tasks: Briefings on the background, rationale, and context of assigned tasks to enhance understanding and effectiveness.

Learning Outcome:

\n

By the end of the academic internship, the student will be able to:
- Conduct client assessments to identify needs, strengths, and barriers to accessing services.
- Develop and implement individualized service plans in collaboration with clients and interdisciplinary teams.
- Utilize effective communication and advocacy skills to connect clients with appropriate resources and support networks.
- Demonstrate proficiency in maintaining accurate and confidential client records in compliance with agency policies and regulations.
