PLEASE READ BEFORE PROCEEDING

This form is used to initiatate the service-learning site registration process. It may be used by faculty wishing to add a service opportunity for their class or a student.

Due to CSUF policy and in accordance with current state and local Safer at Home restrictions in response to COVID-19, ALL Summer 2020 service-learning MUST BE EXCLUSIVELY VIRTUAL (telework/remote).

Students: Consult your professor about appropriate service sites before completing this request to ensure that your proposed service opportunity is suitable.

To proceed follow these steps:

Step 1. Verify the site's registration status with CICE. Some organizations may have registered and posted opportunities in previous semesters. To avoid unnecessary work and time delays, please contact CICE first to verify your organization's current registration status prior to completing this form.

Step 2. Complete this Form. If CICE confirms that your site and/or your service opportunity is not already registered, proceed with this form.

Step 3. Follow-up with the site supervisor as to their progress in completing their registration. They will have received an instructional email from CICE upon completion of this form, but may have missed or skipped it in their inbox. Please ensure they have received and responded to CICE instructions.

PLEASE DO NOT SUBMIT YOUR REQUEST MORE THAN ONCE.
