Program Information Hospice program provides physical, emotional and spiritual support to terminally ill patients and their family members. The program's objective is to educate, empower and support the client and family to live each day as fully as possible, and to manage client's pain to enable them to die with grace, integrity and digniity. Services are available to Monterey and San Benito County residents.
Activities include:
patient companionship visits, community services clinics, administration, visiting with hospice patients, helping in the office, witnessing advanced healthcare directives, community events, assisting with our community services group
job shadow Medical Social Workers, RN's and Nurse Practitioners; collaborate with Interdisciplinary Team by attending weekly IDG meetings; visit and provide companionship visits to our hospice patients and families located at home or skilled nursing facility; participate in Case Conferences for patients and families; administrative support; assist with community outreach and recruitment.
Support the work being done with Palliative Care as well as the Connections Program. Some of the duties include making home visits, educating families on importance of advance directives, work alongside Medical Social Workers and MDs.
Under supervision of the Volunteer Coordinator, CHHS student will provide emotional and psychosocial support to Hospice patients and their families. The intern will be matched with a family and visit the patient/family in their home or facilitiy to provide needed services such as companionship, light massage, emotional support, reading, or respite for the caregiver. Students may also assist in developing our veteran program, to match veteran patients with veteran volunteers and provide training to volunteers on veteran issues. They also provide support to volunteer program in recruiting and training volunteers throught Monterey and San Benito Counties.
Possible capstone might include a recruitment program to increase the number of volunteers, assist in tranining, and increase capacity to serve clients and their families. Prior to patient visitations, interns are required to complete training scheduled for Sept.27 and 28, 2014. Inter-disciplinary Team meetings on Wed. 8:30-11:00 AM.
We require strong communication skills for outreach and patient contact. A passion to work with the older adults and their family members. Bi-lingual Spanish preferred.
