Organization Description

The nonprofit VNA and Hospice has, since 1951, provided a wide range of home health care and hospice services, making more than 75,000 visits each year in all of Monterey, San Benito, and parts of Santa Clara and Santa Cruz counties. The administration office is located in Ryan Ranch, Monterey, and office locations are in Salinas, Hollister, Monterey and King City.  Care is available to all in need, through Medicare, Medi-Cal, Veterans Administration benefits, Worker's Compensation, private insurance and "sponsored care" for those who cannot afford to pay for their care, within available VNA resources through gifts from the community. 

This hospice program provides physical, emotional and spiritual support to terminally ill patients and their family members.  The program's objective is to educate, empower and support the client and family to live each day as fully as possible, and to manage client's pain to enable them to die with grace, integrity and dignity.

Program Information

CCVNA is a non-profit (75 years of service) providing home health, palliative care, hospice, and community programs. Students assist in all programs based on their interest. Specific needs currently are around our ECM program and Aspire Health@Home Provider Visits. Additional opportunities lie within our hospice volunteer and bereavement programs.

Project management / Coordination with our ECM program. Assisting the MSW providing services to CCAH ECM members by data entry into the EMR, coordination of scheduled visits, updating excel spreadsheets. With the Aspire Health@Home project, strong technical knowledge of Excel is needed. This project requires coordination with Aspire members for scheduling physician visits (virtual and in person), maintaining communication with Aspire payor plan, and more.

Strong communication skills, strong computer literacy skills specific to Word, TEAMS, and Excel. Desire around learning more about public health.

Services are available to Monterey and San Benito County residents.

Activities include:

patient companionship visits, community services clinics, administration, visiting with hospice patients, helping in the office, witnessing advanced healthcare directives, community events,  assisting with our community services group

job shadow Medical Social Workers, RN's and Nurse Practitioners; collaborate with Interdisciplinary Team by attending weekly IDG meetings; visit and provide companionship visits to our hospice patients and families located at home or skilled nursing facility; participate in Case Conferences for patients and families; administrative support; assist with community outreach and recruitment. 

Support the work being done with Palliative Care as well as the Connections Program. Some of the duties include making home visits, educating families on importance of advance directives, work alongside Medical Social Workers and MDs.

Under supervision of the Volunteer Coordinator, CHHS student will provide emotional and psychosocial support to Hospice patients and their families. The intern will be matched with a family and visit the patient/family in their home or facilitiy to provide needed services such as companionship, light massage, emotional support, reading, or respite for the caregiver. Students may also assist in developing our veteran program, to match veteran patients with veteran volunteers and provide training to volunteers on veteran issues. They also provide support to volunteer program in recruiting and training volunteers throught Monterey and San Benito Counties.

Possible capstone might include a recruitment program to increase the number of volunteers, assist in tranining, and increase capacity to serve clients and their families. Prior to patient visitations, interns are required to complete training scheduled for Sept.27 and 28, 2014. Inter-disciplinary Team meetings on Wed. 8:30-11:00  AM.

We require strong communication skills for outreach and patient contact. A passion to work with the older adults and their family members. Bi-lingual Spanish preferred.
