Organization Description

Community Health Trust of Pajaro Valley was founded in 1998 and serves a tri-county area. Community Health Trust is recognized for its leadership as a convener in the South Santa Cruz County area. CHT is motivated to foster an equitable Pajaro Valley with health, wellness, and a full quality of life for all through access to resources, activities, and choices that help prevent illness and improve quality of life. 

Program Information

CHT achieves this via 

  • Grantmaking- awarding meaningful funding to agencies across the health spectrum through annual and multi-year grants, endowed funds and scholarships awarded to students in the health and wellness sector.
  • Programs- providing direct services to the community, i.e. our Diabetes Health Center, El Mercado (farmers’ market), VeggieRx (“prescription” food program) and Community Gardens.
  • Collaboratives- identifying issues and bringing community partners together to address and take action on solutions, i.e. our Food, Farming, and Health Policy Council.
