Program Description:

Multidisciplinary (social work, nursing, physical and occupational therapy, nutrition, psychological and recreation) day health setting primarily serving frail elders and adults with disabilities in the Southeast sectors of the city.  Agency also operates the Dr. George W. Davis Senior Center and a Senior Ex-Offender Program, and is actively promoting the development of an Aging Campus which will have affordable supported senior housing with services on site.

Clientele Served:

Seniors and persons with disabilities who have two or more chronic or acute conditions which require regular treatment and monitoring are our target population.    Program orientation is Afrocentric and we serve a multicultural population.

Issues Program Addresses:

Social workers address psychosocial, practical, housing, benefits, aging, living with disabilities and caregiver concerns with individuals and/or small groups.

Intern Assignments:

Intake, assessment, treatment planning, case management, community outreach.  For the academic year 2011-12, additional assignments may include helping people no longer eligible for ADHC services to obtain needed services or programs, secondary to the state’s removing it as a MediCal benefit, and the necessity to “re-tool” our services and attract other funds to continue to serve the community.  Fundraising efforts may be included.

Intern Training & Educational Opportunities:

Very seasoned and culturally competent  social work staff working together; ongoing supervision and participation in any appropriate and available training conferences.
