Program Description:
Case management provides long-term services to seniors who are at risk of premature institutionalization. Case Management helps and empowers low-income seniors with multiple health issues to remain healthy and independent in the community as long as possible.

Primary Issues that the Program Addresses:
Language (illiteracy and Spanish monolingual) and cultural barriers to a wide array of critical services such as transportation, housing, benefits, in-home support services, meals and medicine.
Isolation, Loss and Depression.
Unnecessary Hospitalization.
Premature Institutionalization.
Wellness and health promotion.
Healthy aging

Populations and Clientele Served:
Case Management provides services to seniors 60 years old and older. Most of the seniors are Latinos (Spanish monolingual), Caucasians follow by Asians, and African-American who need ongoing care coordination in order to be safe in the community.

Types of Services Offered by the Program:
Case Management helps seniors to access and maintain services such as Medi-Cal, IHSS, Social Security Income, CAPI, CALM, Lutheran Care for the Aging, Self-Help for the Elderly, Transportation, HDM, Health Related Support, Senior Housing, Cal-Fresh, Dental Care, Phone Discounts, energy discount, etc. Case Management find resources for specific needs: mental health, eviction, private pay in home-support, immigration issues, legal issues. Case Manager ensure safety of client by making referrals to Adult Protective Services (APS) as needed, find resources for Durable Medical Equipment (DME), check for safety in the home during home visits.

Intern Assignments and Learning Opportunities:
Assignments:
-Carry a cased load of 5 clients per semester (total of 10 clients).
-Complete initial/re-assessments, and annual assessments during home visits in the timely manner.
-Complete care plans in the timely manner.
-Monitor client monthly
-Participate and possibly facility staff meeting.
-Participate and present a case per semester in the monthly consultation group meeting.
Learning:
-Intern will learn about the different components of case management (initial assessment, re-assessment, annual assessment, care plans, monitoring, progress notes).
-Intern will learn about the different services available for seniors in San Francisco.
-Intern will learn how to assist and support at risk isolated seniors in the Senior Center as well as homebound.
-Intern will learn how to make reports to Adult Protective Services to ensure safety of seniors.
-Intern will have the opportunity to learn how to present a case and ask for feedback to other social workers with more experience.
-Intern will have the opportunity to learn how to facilitate a staff meeting.

Other Specialized Training and Educational Opportunities:
-Intern will access to trainings, such as Elder Abuse, HIPAA compliance, working with LGBT seniors and other ones.
-Intern will have the opportunity to attend trainings offered by Department of Aging and Adult Services and Case Management Clinical Collaborative Meetings.
