The parent agency, Aging and Adult Services, a Division within the San Mateo County Health System, is comprised of the following integrated services: In-Home Supportive Services, Multipurpose Senior Services Program, Linkages, AIDS Case Management, the HOME Team, the Public Guardian, Community Programs Services Unit, Commissions on Aging and Disabilities, Area Agency on Aging, and Adult Protective Services. The placement is with the Long Term Support Service Project.
Clientele Served:
San Mateo County Aging and Adult Services is an integrated division serving clients 18 years and older (IHSS also serves disabled infants and children) with significant functional limitations due to chronic physical, medical and mental health problems and disabilities. Despite the various categorical funding and eligibility requirements of programs, we provide a continuum of care to our adult community through: In-Home Supportive Services; Adult Protective Services; Representative Payee; Multipurpose Senior Services Program (MSSP); Linkages; AIDS Case Management; the HOME Team; Public Guardian, and multiple services at community-based organizations throughout San Mateo County contracted through our Commissions and Community Based Services unit
Issues Program Addresses: It has been a long-standing goal with San Mateo County to create a consumer-centered continuum of care that would significantly improve service delivery to Medi-Cal eligible seniors and persons with disabilities. This will require the integration of services including home and community-based services, acute services, and long-term institutional services across a broad spectrum of programs and stakeholders. This will be a radical and challenging project and has the potential to improve beneficiary access and experience as well as to produce cost savings for the overall delivery system.
Intern Assignments:
During the initial weeks of placement the intern will be introduced to the history and intent of LTSSP and learn how to conduct assessments using the electronic “uniform assessment tool” (UAT). The intern will have a scheduled comprehensive orientation with all the programs within Aging and Adult Services, Senior Mental health, Ron Robinson Senior Care Center and Burlingame Long Term Care. After or during the orientation the intern will be conducting LTSSP client assessments, learn to interpret the scales and clinical assessment protocols from the UAT to develop care plans, and conduct case management activities. In addition, interns will shadow staff, make home visits, interview clients, collaborate with other professionals, and become introduced to formal and informal supportive services within Aging and Adult Services and the community. When the opportunity exists, attend care planning meetings to participate and present their clients care plans.
Intern Training & Educational Opportunities:
Division wide trainings are scheduled throughout the year. Interns may attend those and others outside the agency relevant to their learning goals.
Intern Benefits: (Include stipend, conferences, car, food, etc.)
Use of county car when scheduled ahead of time. We are NOT able to reimburse mileage to interns who use their personal car in the field.
Other Special Placement Procedures or Requirements (Including Dates of Internship, Special Application Forms and Deadlines): Finger printing of intern required. Recent TB screening. Valid CA Drivers License. Completion of the San Mateo County Aging and Adult Services Application For BSW/MSW Internship (Can be sent electronically upon request).
San Mateo can accept any combination of students – we can accommodate three interns in total.
