Program Information

This placement is with UC Davis Hospice Program, working with patients who have been given a terminal diagnosis with a prognosis of less than 6 months to live.  It involves working with patients and their families and care givers in their home settings.  The social work intern trains with the multi-disciplinary team to provide counseling, support, crisis management, case management, initial and ongoing psychosocial assessments, bereavement assessments, with a population of patients as diverse as the general population in the greater Sacramento area.

Personal vehicle is required.
