Program Description:
The Tom Waddell Urban Health Clinic and Integrated Medical Services provide integrated behavioral health services within a primary medical care and urgent medical care settings. We also provide medical respite care to the most vulnerable adult populations of San Francisco, serving as a safety net clinic for the City and County of San Francisco.
Patients served are primarily homeless or formerly homeless adult patients who have lived in extreme poverty and have been repeatedly trauma-exposed. These patients often have chronic, co-occurring health, mental health, and substance-related problems. In addition to medical services, Tom Waddell Urban Health Clinic also provides a range of psychosocial and behavioral health services, including transgender mental health services including Gender Affirming Surgery assessments, Hep C treatment program, Office Based Opiate Treatment (OBOT) program, Latina focused support groups, Latino male Seeking Safety groups, Integrated Pain Management Program (IPMP) and a variety of other groups and specialties. We work in multi-disciplinary teams include doctors, nurse practitioners, nurses, psychiatrists, behavioral health clinicians, community health workers, medical assistants, and eligibility workers.

Tom Waddell Urban Health Clinic provides a range of psychosocial and behavioral health services to support the safety and health of their patients. Issues addressed include homelessness, mental health, behavioral health, substance abuse, medical literacy, transgender issues, care coordination, advocacy, legal and documentation issues. |

Types of Services Offered by the Program:
Tom Waddell Urban Health Clinic provides a range of primary medical care, urgent medical care, respite care, and integrated behavioral health services to support the safety and health of their patients. |
Intern Assignments and Learning Opportunities:
Under the supervision of the assigned field instructor, the student will be oriented to and trained in best practices in providing integrated behavioral health services to adult, dual and tripple diagnosed patients in a public health, primary care clinic, urgent care clinic, and medical respite. These services include: • Performing comprehensive psychosocial assessment on patients • Providing psychosocial/emotional support to patients and families • Developing and implementing, in collaboration with other team members, a safe and timely care plan and arranging for appropriate community services that meet the patients' needs • Making timely referrals to hospital and/or community agencies and clinical services such as substance abuse treatment programs, and including referrals and reports required by mandatory reporting laws such as child and elder abuse • Participating in clinic performance improvement projects • Providing appropriate documentation and record keeping |
Other Specialized Training and Educational Opportunities:
Interns are supervised closely by the behavioral health clinician (BHC) on site and trained in behavioral health in a public health setting. In addition to being trained in biopsychosocial assessment, care planning, and coordination with multidisciplinary teams and other agencies, interns will be invited to participate in agency quality improvement projects, patient education and wellness projects, and pain management programs, and other health related committees. Our agency is able to offer a very rich selection of training and education opportunities outside of the clinic. For example, interns will also be enrolled in multiple DPH trainings pertaining to service provision in the field of public health and mental health. Interns will be invited to attend regular case conferences at our clinic as well as at SFGH and UCSF departments of psychiatry, medicine, and HIV care. In addition to training in best practices in integrated behavioral health service provision, interns will be trained in current electronic medical record keeping and other relevant systems. |
