Program Description:

For 30 years, Community Health Resource Center has supported the health and well-being of individuals and families in the San Francisco Bay Area. As a non-profit organization, CHRC provides a range of beneficial health education programs and services.

Primary Issues that the Program Addresses:

Community health screenings

Educational lectures

Health information and local resources

Employee and group wellness presentations

Support groups

Populations and Clientele Served:

Patients referred by medical staff: Primary Physicians, Cardiac Rehab, Cystic Fibrosis Clinic, Ray Dolby Brain Center, Brian Hemmings Cancer Center, and walk in clients welcome. We service a wide variety of clientele but specialize in health care settings and with clients ranging in age from 18-90 years old.

Types of Services Offered by the Program:

Behavioral/Mental Health Services

Our licensed team of professionals offer support on a wide range of needs. Are you looking for emotional or practical support? Our services are available for individuals, groups and families looking for emotional or practical guidance and support.

Appointments with social workers are structured in fifty-minute sessions and are sliding scale fee based. To determine the cost of an appointment please see our pricing information, click here.

Counseling session may include:

An Individualized Needs Assessment:

This assessment will help to clarify and prioritize your most urgent concerns in order to develop goals and identify possible solutions.

Short-term Emotional Support:

When faced stress, anxiety, depression or a change in life circumstances, our counselors can assist you by designing a short-term, focused process that is designed to help. Counselors help align resources and make recommendations that can lead to reaching your personalized objectives.

Resource & Referral:

A social worker can help you connect with other helpful resources and agencies in the community as appropriate. This can include connecting with insurance, housing, reduced billing options for utilities, transportation, as well as a wide range of specific community supports.

Psychotherapy:

With well over three decades of experience between the psychotherapists at Community Health Resource Center, they bring their skills and experience to assist your individual needs. The process is a comprehensive, multi-theoretical approach to support you and your well-being. The duration of the support can range and is based on individual needs. Community Health Resource Center takes insurances, including Medicare.

Follow-Up Support:

A follow-up appointment with a social worker can be available depending on your need or interest. Ongoing support may be available depending on availability.

Cancer Buddy Program

Cancer Buddy Program: The Cancer Buddy Program connects patients within the first year of a cancer diagnosis with trained volunteer cancer survivors. The Buddy Program is a forum for individuals to speak openly with someone who understands cancer from the inside. To request a Buddy or to volunteer, call (415) 923-3155.

Stroke Survivor Support Group

This group is designed to aid the recovery of stroke survivors at any stage by providing a safe and supportive atmosphere where individuals are able to share their experiences. This group is made possible by the philanthropic funds of Home Instead Senior Care San Francisco. This group meets the first Thursday of each month from 1:30 – 3:00 pm at 2100 Webster St, Room 119.

Intern Assignments and Learning Opportunities:

Placement in the Family Health Center clinic focusing primarily on case management support for patients needing assistance with housing, financial difficulties, management of chronic illnesses, etc . Additional experience include seeing psychotherapy clients and/or providing resource referrals.

Other Specialized Training and Educational Opportunities:

Opportunity to shadow and assist with the Cystic Fibrosis Clinic and Solid Organ Transplant at California Pacific Medical Center.
