Organization Description

The Latino Diabetes Association provides free diabetes prevention and evidence based self-management education, information, and resources primarily to the underinsured and uninsured, with a focus on the low-income, high risk Latino community.

The Latino Diabetes Association is committed to delivering culturally relevant diabetes prevention and self prevention and self - management education in the community.

Program Information

CORE PROGRAMS: Diabetes Prevention Program, "Lucha Againt Diabetes" (Fight Against Diabetes) This 8 week community diabetes program is desinged to raise awareness on lifestyle,genetic, and environmental factors which contribute to diabetes and proactive steps necessary to prevent its onset or prevent/reduce complications. Taught primarily by Certified Diabetes Educators and Nutritionists or R.D.s, our classes are open to anyone specially low income , high risk persons of any age and ethnicity whether they have been diagnosed with diabetes or not. Also includes Yoga, Zumba, and healthy cooking demonstrations. Diabetes self-Management Program "Standford University Diabetes Self-Management Education Evidence- Based Model" Our 6 week evidence-based program for diagnosed diabetes and/or their family members and caregivers is geared towards diabetes control and compliance, support, medication compliance, exercise and eating healthy, and self-empowerment. to help diabetics that through making gradual lifestyle and behavioral changes, they can live healthy, happy and normal productive lives. Other essential programs: supportive services: the LDA connects our participants through information and referral to a variety of supportive services resources to overcome barriers to health and improve their quality of life such as free and low cost health care, medications,housing, housing, senior services, disabled resources, job information, family services, food pantries, utility assisstance, domestic violence, etc.Exercise : Our "Mi Vida Yoga" Latina-inspired yoga classes to help reduce stress, lower glucose levels, lose weight, and motivate lifestyle changes in diabetics and high risk individuals. Healthy cooking : our "Sabor Y Sazon" healthy diabetic friendly healthy cooking classes are designed to encourage people to eat and live healthier using cost effective healthy versions of their favorite latino and other foods. Diabetes Peer to Peer support groups: these group meetings are held bi-monthly for both diabetes and family-caregivers to deal with the often unaddressed emotional toll of combating and living daily with diabetes.

Background check is not required if community partner supervisor is in classroom or off-site at all times . TB test is required
