Program Description:

IHT is an adult behavioral health treatment program, which provides outpatient behavioral health services to residents of Sonoma County who are eligible for specialty behavioral health services.   The majority of the clients served in IHT are referred from the Access Team or other county behavioral health programs.  Services are provided by a multidisciplinary team working in collaboration with community providers, contracted community partners and other agencies to support clients in accessing resources and supports to move toward recovery goals and objectives. 

Primary Issues that the Program Addresses:

The provision of integrated health care is a key component of the treatment for the clients receiving services from IHT.  Many of the clients have significant comorbid health conditions and working collaboratively with primary care providers and with the Bridges to Health program we are striving to provide early intervention and prevention and to address health conditions that lead to early morbidity for consumers living with serious mental illness.

Populations and Clientele Served:

The IHT serves adult residents of Sonoma County who are dealing with serious and persistent mental illness.  This program maintains a census of approximately 360 clients referred through the Access Team or other Sonoma County Behavioral Health teams.  The IHT employs a Spanish speaking PSC (case manager), Psychiatrist, RN and clerical staff.  All clients receive culturally sensitive services and we strive to provide client-centered, recovery oriented treatment.

Types of Services Offered by the Program:

Clients in the IHT program receive:  psychiatric assessment and treatment, psychosocial assessment and planning, psychiatric RN support, monitoring and medication management and coordination.  In addition, clients receive targeted case management, individual and group therapy and crisis intervention as needed.

Other Specialized Training and Educational Opportunities:

Interns have opportunities to participate in on-going staff development trainings on a variety of topics, including Recovery Model, Cultural Humility, Law & Ethics, and Safety.  Additionally, interns have the opportunity to learn about specific treatment models such as DBT and Psychiatric Rehabilitation Approach.

Prior Behavioral Health/Crisis experience is desired.  Spanish-speaking is highly desirable.  Any multi-language ability is valuable.  Dual-Recovery knowledge is valuable.  Familiarity with Medi-CAL charting requirements is helpful.  Familiarity with AVATAR is helpful.  Lived-experience is welcomed.
