Program Information EmpowerCare is a Geriatric Care Management Practice that was established 8 years ago. As a provider of creative care management and personal care services, we aim to bridge the gap between an aging population of seniors and the loved ones who take responsibility for their care. Through a comprehensive assessment, out proprietary approach aligns the resources and support needed to reduce stress, rising costs, avoidable hospitalizations while improving their quality of life.
