Organization Description

The mission of the Foundation for Senior Care (aka Fallbrook Healthcare Foundation) is to provide programs and resources to Greater Fallbrook area seniors and disabled adults, helping to enhance their well-being and live a more meaningful life. Our programs fill a critical gap in health and well-being while providing affordable options for low-income families. Initiatives of the Foundation for Senior Care (FSC) include transportation services, adult day programs, advocacy, referrals, case management, and technology education for seniors (age 60+) and adults with disabilities in the greater Fallbrook area.

Our Resource & Referral Advocates have backgrounds in social or human services and directly help seniors with a wide variety of services, including case management for some. Through our separate Adult Day Care program, another team provides caregiving and socialization for adults with Alzheimer's, dementia, stroke, Parkinson's.
