Organization Description

Liberty Towers Church in North Highlands Foothill Farms mission is to transform the community. It is well known for diversity and inclusion as well as it's innovative approach to outreach and community engagement. It is best described as a community center during the week that offers a worship service on Sundays. During the week, Liberty Towers hosts a variety of workshops, classes, and events for all age groups on and off campus. Liberty Towers also serves as the Community Incubator Lead (CIL) for North Highlands / Foothill Farms Black Child Legacy Campaign. As the CIL, a Sacramento County Multi Disciplinary Team (MDT) is housed at Liberty Towers to assist with coordinating systems of support for families and individuals in the under-served community. It is an organization deeply entrenched in the community and partnered with influential, long standing, reputable institutions including Twin Rivers Unified School District, affordable housing communities, community based organizations, local stakeholder and elected officials, and others to equip students, individuals and families with tools to thrive, become stronger and /or more proficient in their area of need.

Program Information

Formal and informal case management, serve as a member of the MDT, community outreach and networking, attend community meetings, events and training, facilitate courageous conversations, possibly host workshops, classes and table at community gatherings. Document interaction and progress notes, with accurate, and timely data entry. Work in teams and independently on various tasks and projects; share your honest insight, feedback and perspective. Genuinely communicate with children, teens, adults and seniors to address their needs and concerns, without judgement. Serve as a navigator or advocate, and connect to community based organizations and serve providers. Utilize skills and strengths to enhance programs, resources and services. Work with the team to plan and implement best practice approach to development, relationship, and community building strategies. Do your best to provide quality customer service and support to individuals and families. Assist with running programs and activities.

Health & Safety

Because we serve the entire community, we are uncertain who may come to the office to seek services. However, our building is locked and individuals or community members have to be given access into the facility.
