Dignity Health: Yolo Adult Day Health Center

The student will be a member of the social work department and participate in all departmental responsibilities. Training will focus on initial and ongoing psycho-social assessments; development of care plans; daily documentation; application of clinical tools; complex care coordination, group work, home visits, community collaborations and program administration (if interested) with emphasis on community-based chronic disease management. Additionally, adult day health care offers the opportunity to work within a dynamic onsite interdisciplinary team.
