Organization Description

Health Outreach Partners’ mission is to build strong, effective, and sustainable grassroots health models by partnering with local community-based organizations across the country in order to improve the quality of life of low-income, vulnerable, and underserved populations. Our vision is a country in which all people are valued and in which equal access to quality health care is available to everyone, thus enriching our collective well-being.

Program Information

• Plan and facilitate regional site-specific, and distance trainings
• Develop training curricula and supporting materials
• Plan and facilitate intensive consultation services, to include outreach program assessments, outreach program development consultations, and other facilitated planning consultations.
• Coordinate and contribute to HOP’s publications and website, to include: strategy and vision, communication, writing, editing, and publication design.
• Develop information resources to support outreach programs, including reports, e-alerts, fact sheets, and other products.

By the end of this opportunity, students will have learne program planning, development and implementation of training, and technical assistance for Community Health Centers and FQHCs throughout the United States.
