River City Medical Group

Organization Description

River City Medical Group manages the care of Medi-Cal and Medicare beneficiaries and acts as intermediary between providers, patients and health insurance plans. Our Program Development department hosts case managers who are stationed within community medical clinics across Sacramento and work to link patients to community and entitlement resources as well as navigate referrals and physicians' orders through the health plan.

Program Information

Students will get experience with medical case management. Working closely with patients, doctors, health plans and other medical provision entities. Students will assist patients in reducing barriers to healthcare and will help improve health outcomes.

Health & Safety

Due to the current pandemic all social workers and navigators are currently working from home and conducting telephonic case management with rounds via video and telephone three times per week. However, when it is necessary for employees or students to go to medical clinics, hospitals or skilled nursing facilities PPE is provided and CDC recommended guidelines are followed.

  • Computer Literacy
  • Driver's License
  • Must be 18 or older
  • TB Test
  • Fingerprinting or Livescan is Required
  • Personal Vehicle Required
