Driving innovation to improve the health and well-being of older adults, support vulnerable populations, and address critical unmet needs, Determined Health empowers organizations to effectively address social isolation and other critical social determinants of health. Through our Connection1st CallHub, we connect volunteers with socially isolated older adults, individuals with disabilities, caregivers, and others with complex care needs, to build meaningful engagement. Program Information

Determined Health’s Connection1ˢᵗ approach addresses social determinants of health by building meaningful connections. We envision a new way to live and age well—one that places human connection at the center of care. People are the missing link, and relationships and trust are key ingredients for effective, equitable care. Although the scale of the problem can feel overwhelming, evidence shows that tackling loneliness is one of the most powerful places to start. Increasing social connectivity—a critical determinant of health and a fundamental human need—improves outcomes across the lifespan. Building on our experience helping programs nationwide combat social isolation and loneliness, we’ve developed an end-to-end solution that enables individuals and organizations to mobilize and create meaningful social connections—the bedrock of good health and well-being. All students, regardless of race, ethnicity, national origin, religion, sex, gender, gender identity, sexual orientation, disability, or socioeconomic background, are welcome to participate.

Health & Safety

Telephonic engagement for frail and primarily homebound older adults and individual with disabilities

We provide telephonic social connection to older adults, individuals with disabilities, caregivers, and other groups at higher risk of social isolation.NAFollowing initial social call program volunteer training along with informational sessions providing background on central topics (eg social isolation, loneliness, social determinants of health, healthy aging, community health, health system innovations/challenges, etc.) students will conduct social telephone calls through the Connection1st CallHub system (which tracks all call activity). Students will then participate in individual and group supervision, in which they will have an opportunity to process their experience making social calls, strengthen social call skills, work-through challenging situations, address any potential concerns, etc.

Direct supervision by project team leaders and overall supervision from clinical and organizational operations leadership
