Program Information

Interns will have the opportunity to assist with community health promotion and prevention services, including diabetes prevention and condition management services and substance use and overdose prevention. This includes engagement with the delivery of diabetes and substance use education in school sites across Monterey County, program promotion in the community, and telephonic outreach to identify needs and connect people with appropriate health and community resources. This opportunity will provide diverse health promotion experience working with a variety of populations.

General Description of what students will be doing at this Site: Interns will have the opportunity to assist with diabetes prevention services and community health education including the delivery of diabetes education in school sites across Monterey County and telephonic outreach to identify needs and connect people with appropriate health and community resources. Key Responsibilities: 1. Provide support to all condition management and wellness initiatives 2. Facilitate diabetes education modules to students at school sites across Monterey County 3. Assist with administrative duties including, but not limited to, manual entry of member information into database, sending health education materials, letters, and community resources, organization of the department, etc. 4. Interact with members telephonically and in-person to engage members in programs and services 5. Diligently track interactions with members in appropriate data portals 6. Work collaboratively with Aspire Health’s multi-disciplinary team to meet member’s needs and provide excellent customer service. 7. Other duties as assigned
