Organization Description

The mission of Catholic Charities is to provide specified human services to the poor and disadvantaged, to promote justice, peace, human dignity, self-determination and self-sufficiency in accordance with the gospel and Catholic social justice teachings, and to cooperate with other individuals and groups of goodwill to strengthen our communities.

Catholic Charities of the Diocese of Monterey, is a faith-based non-profit social service agency providing aid to individuals and families in the four central coast counties of Monterey, Santa Cruz, San Benito and San Luis Obispo.

We are motived by our mission to assist individuals and families of all backgrounds and beliefs providing them with tools, training and resources to help meet basic necessities for life. In addition, we provide information and referrals to social service agencies in each community we serve. Our three core areas of service are Immigration and Citizenship, Family Supportive Services, and Disaster Relief. We serve over 20,000 individuals a year. All services are offered bilingually.

Program Information

Clinical Case-management
Through Pathways to Stability deliver services to support participants in moving from unstable financial situation to a more secure base. Some Essential
Duties and Responsibilities are:

1. Answer and log-in phone calls; clear voicemail messages, and greet visitors.
2. Conduct client needs assessment.
3. Assist clients with the application process to Hope in Home, Pathways to Stability, CalFresh and MediCal applications.
4.Provide long term clinical case management for participants enrolled in Pathways to Stability.
- Support clients in setting individualized goals for stability.
- Follow up, motivate, and support clients in achieving their goals during the 6- 9month program participation.
5. Make appropriate referrals to community and government agencies.
6. Prepare complete client file in AWARDS client database.
7. Plan and deliver educational workshops & support groups.
8. Attend outreach events in the community.
9. Assist the Director of Family Supportive Services with program needs and help identifying potential process improvements.
10. Attend staff meetings and training as needed.
11. Attend case discussion and group supervision on Tuesdays 10:30am to noon.
12. Complete a minimum of 16hrs a week on placement.
13. Other duties as assigned by the Director for Family Supportive Services.

Note: placement and supervision will be at our Seaside office, but occasionally, students might need to travel to other locations such as Salinas, Soledad or Greenfield for client appointments.

Optional:
Assist the Program Director with research and data analysis during the grant writing process.

Health & Safety

Risk is Very Low. Target population is outpatient and mainly have situational issues.

Be cautiously aware. Open to learning, ask questions, open to all supervision. Willing to learn from mistakes as well to interpret success within the learning process.
