Community Hospital of the Monterey Peninsula is a nonprofit healthcare provider serving the Monterey Peninsula and surrounding communities. It has grown and evolved in direct response to the changing healthcare needs of the people it serves.
Program Information Placement 1: Inpatient Psychiatric Unit-Garden Pavilion: 18 bed unlocked adult acute psychiatric inpatient unit. Psychiatric treatment for acute conditions involves stabilization, medication management, therapeutic interventions, and group treatment. Social workers provide the bio psychosocial evaluations for patients, run groups, meet with patients, hold family meetings, attend treatment team, and provide discharge planning and resources.
Placement information: 2nd year of placement students with acute hospital, medical and/or psychiatric experience. Hours: Day hours 4-8 hour shifts, Days of the week: negotiable, Monday-Friday, Saturdays possible.
Placement 2: Outpatient Behavioral Health Services: Outpatient Behavioral Health-MSW student would work in conjunction with psychiatric nurse practitioner with patients recently discharged from inpatient psychiatric hospital or medication management patients. MSW may meet with patient/family to assist with: resources, completing information, making referrals, education, transitioning patient's into appropriate care in the community, etc. (not individual therapy!). Motivational Interviewing skills are helpful. Opportunities to assist with Groups and Classes and other community projects, (community health fairs, talks, etc) may be available.
Placement Information Prefer 2nd year of placement and students with group and psychiatric experience. Student should be a self starter, organized, maintain confidentially, be detail oriented and able to identify opportunities for social work interventions. Work hours negotiable, Monday-Thursday 8am-5pm Medication clinic is 3pm other hours may be available with classes, or other community projects. Not at CHOMP Hospital; in Monterey office
Placement 3: Social work Department-Medical Social Work
Social workers see referred patients in the hospital for screenings, psychosocial evaluations, identifying and providing referrals and community resources . Social work covers various areas within the hospital including telemetry, family birth center, ICN, ICU, ED, CDU, and med surg units. Medical social workers assess to appropriate resources, coordinate referrals and varied interventions. Will be involved with multi disciplinary team meetings and abuse team meetings. Placement Information: Candidate will be interested in working in hospital environment: fast paced work, high client turnover -short term relationships, would prefer a fast learner with previous SW experience, 2nd year of placement students only. Work Schedule: Generally Tues/Thursday but negotiable & weekends to be considered (after training). Prefer 8 hour days Hours: 8 _ 4pm.
Placement 4: Cancer Center
Social work will be working with patients in hospital as well as outpatients coming for chemotherapy and other cancer care. Student will have opportunities to work with patients as well as families and caregivers. Assist with various cancer support groups, work with interdisciplinary team, provide psychosocial assessments, interventions and support.
Placement Information: Candidate will be able to work with the impact of debilitating diagnosis, able to work with multiple disciplines. Monday _Friday days and hours negotiable.
Placement 5: Palliative Care Inpatient Hospital
Social workers see referred patients in the hospital for screenings, psychosocial evaluations, identifying and providing referrals, and provide patient and family psychosocial support.
Placement Information: Candidate will be interested in working in hospital environment: fast paced work, high client turnover -short term relationships, would prefer a fast learner. Ideal student will have previous SW experience with knowledge of SW code of ethics. Work Schedule: Weekdays; days negotiable but Wed/Thurs preferred. Hours: 8:30 _ 5pm.
Placement 6: Transitions Program for Hospice of the Central Coast: Transitions is a nationally proven program that supports individuals facing a serious illness and life-threatening prognosis of 18 months or less. Transitions coordinates community services to help the individual and family. The student would work with LCSW coordinating services for clients in the community who are nearing end-of-life, complete psychosocial s, work on identifying plans and goals, facilitate appropriate referrals to hospice and be a liaison for hospice and the community.
Student Qualities: Energetic, excellent communication skills, work well within a multidisciplinary team, able to maintain strict patient confidentiality and be interested in working with end-of-life clients.
Work Schedule: Tues, Wed, Thursdays Not at CHOMP Hospital; Ryan Ranch Offices.
