Organization Description

The nonprofit VNA and Hospice has, since 1951, provided a wide range of home health care and hospice services, making more than 75,000 visits each year in all of Monterey, San Benito, and parts of Santa Clara and Santa Cruz counties. The administration office is located in Ryan Ranch, Monterey, and office locations are in Salinas, Hollister, Monterey and King City.  Care is available to all in need, through Medicare, Medi-Cal, Veterans Administration benefits, Worker's Compensation, private insurance and "sponsored care" for those who cannot afford to pay for their care, within available VNA resources through gifts from the community. 

This hospice program provides physical, emotional and spiritual support to terminally ill patients and their family members.  The program's objective is to educate, empower and support the client and family to live each day as fully as possible, and to manage client's pain to enable them to die with grace, integrity and dignity.

Program Information

We provide referral and coordination of community resources, assistance with advance health care directives and POLSTs, emotional support, provide education to patients on how to communicate with their medical team (what questions to ask, etc.). Some examples of short-term SW needs are making referrals to: MST Rides, Meals on Wheels, Lifeline/ Life Alert medical alert service, private caregivers and caregiving agency resources, referral to mental health services, and many more. Our services are conducted telephonically. We provide 1-2 phone call check-ins a month to patients. When scheduling allows, we make efforts to conduct an in-person visit to assist with advanced healthcare directives and POLSTs.
