Home Avenue is committed to providing high quality health care. Our friendly skilled staff of health care professionals are here to help our participants achieve optimum health so they can “live life to the fullest”. We seek to expand health care and human services to provide for the growing needs of the frail, elderly, and disabled adults. We serve persons with cognitive impairments such as Alzheimer’s, persons with mental health issues, persons with chronic medical impairments, and persons with disabilities.
The populations we serve are low-income adults, age 18+, with chronic medical or mental health conditions, cognitive impairments, behavioral issues, or disabilities. Majority of our population are elderly seniors.
Must follow HIPAA compliance.
Students will be responsible to report to the supervisors of the department they are assigned to. Most assignments should be done independently but staff is always available to supervise or provide guidance.
- Driver's License
- Health Check/Screening (e.g. temperature screening, COVID-19 test)
- Must be 18 or older
- TB Test
- Vaccinations
Office hours: Mon- Fri 8:30am-5pm, Program hours: Mon- Fri 9am-2:30pm
