Organization Description

Telecare’s goals to deliver excellent and effective behavioral health services that engage individuals with complex needs in recovering their health, hope, and dreams. ICT Program: Intensive Community Treatment (ICT) is an adjunct behavioral health outpatient program that serves Adult members treated by the Kaiser Permanente Medical Group psychiatric and addiction medicine programs within Northern California, who meet criteria of 3+ hospitalizations in the previous 12 months thus needing intensive services due to previous patterns of high inpatient care utilization or otherwise being at risk for readmission.

Program Information

Actively learn by shadowing the team: access, enrollment, assessment, intervention design, implementation, and discharge planning. Students will work with individuals to help assess risk, help individuals "build their communities" in the outpatient world, increase the use of coping skills, learn interventions through 1:1 work in the field (member's home, community setting, work site) to improve their quality of life, reduce hospitalizations, reduce high-risk behaviors, and increase connectedness with their primary Kaiser provider.
