Organization Description
The Community Health Improvement Program, through a dedicated interdisciplinary healthcare team, focuses on appropriate patient identification, prevention, education, evidence-based disease management and exemplary treatment of individuals with chronic conditions. CHIP targets medically indigent patients within the service area who have a chronic condition (such as heart failure, asthma and or diabetes), and as a result of difficulty managing their chronic condition and coordination needs, experience frequent emergency visits and hospitalizations. More recently, the program has reached out to other risk populations as the needs of our community evolved.
