Organization Description

The mission of Delta Health Care is to promote health and wellness in our community and provide services that support individuals and families of San Joaquin County in reaching their full health potential.

We fulfill this mission by providing a variety of community services and programs.  Our service delivery system includes WIC supplemental nutrition programs, breastfeeding promotion and support, community education and outreach, school-based health care for teens, and referrals to resources offered by our community-based partners.

Our Vision is to be a health and wellness advocate and source of support for a community that believes in health and wellbeing for all its members.  We envision an informed, proactive community where individuals and families have access to quality health services and wellness programs.
