Prime Hospice care is a healthcare service provider that provides support and comfort for individuals with a life limiting terminal illness. Our mission: To provide respectful and compassionate care to the clients we and prioritize their comfort and health needs. Our goal is to provide guidance and comfort to our patients and their family’s throughout the entire hospice experience. With our skills and combined knowledge, our commitment is making you and your loved ones lives easier.

Program Information

Development of bio-psychosocial assessments with the patient and family members. Identifying the immediate needs of the patient and their family members. Provide grief counseling to patients, family members, and loved ones . Attendance of the interdisciplinary group meetings, assist with the completion of Physician Orders Life Sustaining Treatment (POLST) forms, along with the Advance Health Care Directive (AHCD). Conduct care conferences with team members, family members, and nursing facility staff. Providing conflict resolution and assist with the stabilization of certain family dynamics. Assist with referrals to a variety of community organizations, Medi-Cal, Veterans Administration, and In Home Supportive Services (IHSS).
