Case Manager InternAt — Hope By The Sea

Opportunity Summary 

The Case Manager is responsible for providing direct services and supportive contacts to the client and/or their families, and developing resources within the community, which are supportive to the recovery of the client. He/she also functions as an advocate for the client, seeing that appropriate services are provided and assists the client in receiving these services as appropriate.

1) Provide direct clinical services (i.e. psycho-social assessment, consultation, individual/group/family counseling, professional in-service training, community education services, etc.) 

2) Open and maintain case records.

3) Work cooperatively with other community resources and referral sources to coordinate services to clients.

4) Assisting in the assessment of area needs for mental health, substance abuse, and program services.

5) Good writing skills which enable one to accurately chart client progress and interaction during contact.

6) Communicate clearly and assertively with clients and staff.

7) Lecture in a large and/or small group setting.

8) Ensures safe care to patients adhering to policies, procedures, and standards of care.

9) Promotes individual professional growth/development by meeting requirements for mandatory/continuing education/skills competency; supports department goals which contribute to the success of the organization.

The intern will demonstrate their learning outcomes by:
Completing After Group Reports (AGR) to gain insight into progressions in her role.
Allocating a portion of supervision to focus on understanding of learning outcomes.

Training:

To help the intern achieve the proposed learning outcomes, I will:
Hold supervision sessions weekly for one hour to assist Intern in developing clinical skills.
Schedule Intern to shadow clinicians in group facilitation.
Review an in-depth orientation packet (that describes the role, history, and philosophy of site) alongside Intern and answering questions along the way.
Offer mandatory training on Discharge planning (1 hour), treatment planning (1 hour), and assessments (1 hour).

Learning Outcomes:

By the end of the internship, the student will be able to:
Facilitate a content group utilizing skills and interventions developed through weekly supervision with site supervisor.
Gain familiarity with clinical documentation and maintain 100% timeliness in group documentation submission.
Utilize Motivational Interviewing to assist clients in developing motivation to grow as well as face inner conflict.
Understand information about Hope by the Sea including the importance of role, history of the field/company, and philosophy of the field/company.
