Case Management Intern - Buena ParkAt — Illumination Foundation
The Case Management Intern is compassionate and dedicated to assessing the strengths and needs of homeless clients with a proven ability to build rapport. The most important aspects of providing wraparound services are knowledge of community resources and actively following up with each interaction made while building Relationships.

Potential Role & Responsibilities:

- Assist clients in obtaining vital documents
- Help case management team with client intake
- Support clients in applying for assistance, i.e., Medi-Cal, Disability
- Take on a small caseload of clients to support their journey to self-sufficiency
- Assist Day Hab Program
- Build a community resources list and update the list of current services
- Connect clients to appropriate resources and supportive services.

Tasks:
\n
1. The intern will demonstrate their learning outcomes by, engaging in client interactions and staff guidance and debriefing
2. The intern will demonstrate their learning outcomes by, planning and meeting with the 1-3 clients weekly and follow up with activities.
3. The intern will demonstrate their learning outcomes by, coordinating with community organizations, and documenting client interactions.

Training:
\n
Each of our five sites has a Lead Case Manager who with supervise and mentor interns assigned to their site and will be present during intern's interaction with clients.
\n
1. To help the intern achieve the proposed learning outcome, IF will provide proper training and have the intern shadow conversations, intakes, and observe crisis situations.
2. To help the intern achieve the proposed learning outcome, IF will provide the opportunity to work one on one with 1-3 clients with supervision.
3. To help the intern achieve the proposed learning outcome, IF will share community resources, GIRP style documentation training, and provide de-escalation techniques.

Learning Outcomes:
\n
1. By the end of the internship, the student will learn problem solving skills, active listening skills, conflict resolution skills, and work
2. By the end of the internship, the student will learn to increase verbalization with the specified client population, develop interpersonal skills and collaborate with other organizations.
3. By the end of the internship, the student will learn community resources, learn to write case notes, and learn de-escalation techniques.

1. Security on site 24/7
2. Clients might have warrants and handled in-house, connected to the homeless court. They have typical warrants of Trespassing, Shopping carts, public intoxication, substance abuse related, etc. NO 290's accepted at facilities (sex offenders).
3. Parking at Fullerton is designated at Wings Cafe, and shuttle transport is offered.
4. Safety is our number one priority for staff, interns, volunteers, and clients. Security and video surveillance are available at most sites.
