Organization Description

PK Management's Social Services department promotes positive, successful resident and community development. They currently provide access to over 7,000+ individuals residing on our properties. We have 37+ nationally recognized social services programs throughout 14+ states. Our vision is to develop local and national partnerships in order to improve the quality of life for our senior residents. We work to reduce negative risk factors and remove barriers that may create instability to help them maintain independence and enhance their livelihood. Our team promotes economic, social, and physical well-being for residents through case management, crisis management, advocacy, community partnerships, and other innovative strategies.

Program Information

• Interact with residents, monitoring their conditions, needs and the services they are receiving. • Assessment, reassessments, case management and crisis management. • Develop and manage programs to enhance quality of life and increase cognitive stimulation. • Reporting, documentation, and record-keeping. • Community building; connect residents with community resources. • Make referrals to appropriate agencies. • Develop monthly newsletter and calendar of resident educational programs. • Attend Corporate Service Coordinator meetings and coalitions with Senior Service Providers. • Other responsibilities as assigned by the VP of Community Development; the Director of Social Services/Community Development and/or immediate supervisor.

Health & Safety

Most seniors have received Covid, Flu vaccinations but not all. Covid vaccinations are not mandatory. Students should be aware of resident's utilization of motor operated scooters, wheelchairs and canes.
