Position Overview:

As a Case Management Intern, you will play a crucial role in supporting our clients and clinical staff. You will assist clients in navigating and accessing the resources they need while contributing to various case management tasks. This internship offers valuable hands-on experience in the field of case management and an opportunity to develop professional skills in a supportive environment.

Tasks:

Key Responsibilities:

Client Assistance: Aid clients in identifying and accessing necessary resources, including but not limited to social services, healthcare, and community support.
Resource Navigation: Assist clients in understanding and completing applications for resources and services.

Support to Clinical Staff: Provide administrative and organizational support to clinical staff, including documentation, scheduling, and follow-up tasks.

Communication: Maintain clear and empathetic communication with clients, ensuring their needs and concerns are addressed promptly.

Time Management: Effectively manage and prioritize tasks to meet deadlines and contribute to the smooth operation of the case management process.

Organization: Help maintain accurate and organized records, ensuring all client information is up-to-date and confidential.

Training:

Case management interns will receive training through a comprehensive training and onboarding process, group mentorship, one on one mentor meetings, and shadowing opportunities. Skill development will be assessed by case reports, weekly debriefs, and observation.

Learning Outcome:

By the end of the academic internship, the student will be able to identify clients in need of a higher level of care, conceptualize systemic issues and barriers to stabilization, obtain appropriate resources and referrals, and identify key factors of professionalism in a healthcare environment.
