Case Manager InternAt — Institute for Healthcare Advancement / La Habra Family Resource Center
The La Habra Family Resource Center provides a complete, family centered support system, working with community resources that can address the health, emotional, social, and academic needs of children and their families. The La Habra Family Resource Center provides services to over 3000 people in the community and is involved with a myriad of La Habra community organizations. Our FRC serves low-income and at-risk families within Orange County with children 0-18 years of age with family advocacy, case management, domestic violence services, individual, family and group counseling; legal advice; parenting education, information and referral services and more.
As our Case Manager Intern, you will learn the operations of the Family Resource Center. You will manage families and connect them with services. You will get hands-on experience and working with families directly. You will participate in trainings, staff development activities, meetings and special events. You will work closely with social services agencies and gain knowledge on services offered by outside agencies. We will offer support throughout your internship experience.
Tasks:
Primary duties include but not limited to working closely with Family Support Advocate/Case Manager. Student will immediately shadow case manager and when they feel comfortable, student will take on cases on their own. Student will be responsible to manage case plan and present to our Multi-Disciplinary Case Management Team on services requested by the family. Student will enroll family in Database and type of case notes. Student will complete other duties as assigned
Training:
To help the student achieve the learning outcomes, staff will be available for student whenever they need through text, email, or phone call. We will provide different trainings such as child abuse training, elder abuse training, active shooter training etc. Our internship is very hands on. We will have the student shadow the case manager, and then when comfortable let them do task on their own. Student will be involved in weekly Case Management Teaming to learn about different agencies and the different roles everyone plays. We will check in with students and make sure they are getting their self-care.
Learning Outcome:
By the end of the academic internship, the student will be able to work within the agencies policies and procedures, gain knowledge of the population being served, learn to access appropriate client resources, develop public speaking skills and identify dangerous/crisis situations.
