Case Management Intern - CalAIMAt — Homeless Intervention Services of Orange County (HIS OC)

Opportunity Summary 

Seeking a dedicated Case Management Intern to support our efforts in assisting individuals and families experiencing homelessness. This internship provides hands-on experience in client advocacy, service coordination, and case documentation. Interns will work closely with our case managers to conduct intakes, connect clients with resources, and support them in achieving stability. Ideal candidates are compassionate, detail-oriented, and eager to learn about social services and direct client support.

Tasks

Client Intakes & Documentation

Assist in conducting client intakes, gathering essential demographic and background information.
Complete Individualized Housing Stability Plans (IHSPs) in collaboration with case managers.
Accurately document interactions and progress notes in the case management portal.
CalAIM Services Coordination

Support case managers in assessing client eligibility for CalAIM services.
Assist in linking clients to appropriate Medi-Cal benefits and community resources.
Learn to track and follow up on service referrals.
Case Management Portal Utilization

Input, update, and maintain accurate case notes and client records in the case management system.
Ensure compliance with data entry standards and confidentiality policies.
Client Support & Advocacy

Engage with clients to assess immediate needs and provide referrals for housing, healthcare, employment, and social services.
Shadow case managers during client meetings and home visits (if applicable).
Provide resource navigation support to help clients access available benefits.
Team Collaboration & Professional Development

Attend team meetings, case conferences, and training sessions.
Participate in one-on-one mentorship meetings for feedback and professional growth.
Engage in job shadowing and learning opportunities related to trauma-informed care and crisis intervention.
Administrative & Program Support

Assist with organizing client files, reports, and program materials.
Support outreach and engagement efforts for workshops, job fairs, and resource events.
Conduct research on best practices in case management and service delivery.

Training
To support the student intern in achieving their learning outcomes, the following training and development opportunities will be provided:

General Employer Orientation (Mandatory): Interns will receive an overview of the organization’s mission, policies, and procedures, as well as an introduction to the team and key stakeholders.

Case Management Training: Interns will shadow experienced case managers to learn intake procedures, Individualized Housing Stability Plans (IHSPs), and best practices for case note documentation.

CalAIM Services Overview: A structured training on the CalAIM program, focusing on services available for individuals experiencing homelessness and enrolled in Medi-Cal, including eligibility, benefits, and referrals.

Case Management Portal Navigation: Hands-on training to familiarize interns with the digital case management system, ensuring accurate documentation and compliance with reporting standards.

Weekly One-on-One Mentor Meetings: Regular check-ins with a supervisor or mentor to review progress, address challenges, and provide guidance on professional development

Learning Outcomes
By the end of the academic internship, the student will be able to:
Demonstrate an understanding of CalAIM services available to individuals experiencing homelessness and enrolled in Medi-Cal, including eligibility, benefits, and referral processes.
Accurately complete client intakes, Individualized Housing Stability Plans (IHSPs), and case notes, ensuring documentation aligns with best practices and organizational standards.
Effectively navigate and utilize the case management portal to input, track, and update client information and case notes.
Develop professional communication and advocacy skills when working with clients, service providers, and interdisciplinary teams.
Apply ethical and trauma-informed practices in case management while supporting individuals facing housing instability.
