Program Information

The role is high-touch, in-person, complex case management including completing a comprehensive Assessment and Care Management Plan (similar to a biopsychosocial analysis), providing enhanced coordination of care, health promotion, transitional care services (post ER or hospitalization), member and family support, and coordination of referrals to community and social supports.
