Opportunity Summary 

OC Anaheim ADHC is an Adult Day Health Care facility, which is a prevention facility serving 18+, adults who have special needs. Participants can handle themselves, It is a facility before assistant living, nursing home.

90% of participants are non English social isolate seniors, ethnic groups are Chinese and Vietnamese.         

Tasks:

Complete mock and live assessments under supervision, Draft care plans for assign assigned participant(s), Conduct supervised par participant/family consultation

  • Conducts various assessments, care plan development and advocacy for each participant.
  • Researches and coordinates community referrals.
  • Monitors and reports case recording on a timely basis.
  • Participates/leads in coordination of social/health services.
  • Participates in team meetings and provides consultation to therapists, families and participants.
  • Assists with the Program Director’s day-to-day management of the ADHC.
  • Assists with supervision of staff, audit preparations and reporting.
  • Other duties as assigned.

Qualifications:

  • Majoring in psychology, gerontology, social work, or counseling 
  • Ability to work with individuals with physical or mental limitations;

Preferred Skill:

  • Bilingual English/  Chinese/ Mandarin ,Cantonese Vietnamese

Training:

Orientation on assessment tooling (MMSE, GDS, interview) and shadow social worker (2x/week) review case files, receive feedback after each observation

Learning Outcome:

Conduct comprehensive psycho social assessments for language barrier older adults and individuals with Chronic conditions
