Organization Description

Day Break Adult Day Health Care Center is a state-licensed facility that provides adult day health services to seniors living with chronic illnesses and adults suffering from mental, physical and developmental disabilities. We offer nursing, social and rehab services, as well as nutritious meals, activities, dietary education and transportation. W also offer enhanced care management to at risk populations to assist them with navigating the healthcare system and keep them safe and healthy in the community for as long as possible.
