Job Description - SUD Counselor/Case Manager
MISSION STATEMENT

Our mission is to provide a safe, comfortable, alcohol and drug-free environment for detoxification and provide chemical dependency recovery services to assist clients in obtaining long-term sobriety.
SUMMARY
The SUD Counselor / Case Manager will be responsible for providing services to clients with a primary diagnosis of chemical dependence, providing individual, family, or group therapy, and case management under the direct supervision of an experienced licensed or certified SUD clinician. This position shall be a client care and coordinator of services role, completing government documents and case files, employed in exchange for field experience, while completing school for a certification in Substance Use Disorder Counseling. The Case Manager shall cooperate as a team member in the provision of a therapeutic environment that encourages each client’s emotional growth, facilitates lecture or process groups, counseling, and provides support. The position is expected to maintain an acceptable level of productivity as measured by effectiveness and treatment outcomes, and may be part-time to full-time.

• High school diploma, GED, or one year of work experience in a supportive role.
• Credentials: CADC or higher
RESPONSIBILITIES
\u25cf Uphold and enforce the agency’s standards, policies, and procedures, client rights, and professional code of ethics and conduct.
\u25cf Demonstrate knowledge of TAP 21 Counselor Competencies.
\u25cf Conduct admissions eligibility screening, including presenting problems, critical needs of the client, and determine if the agency is appropriate to meet the individual needs of a client, if required.
\u25cf Ability to utilize agency comprehensive assessment tools (biopsychosocial, ASI, or ASAM-PPC2 Patient Level of Care Placement Criteria).
\u25cf Identify And Prioritize Individual Needs And Problems.
\u25cf Prepare comprehensive Interpretive Summaries per Joint Commission standards.
\u25cf Develop person-centered care plans, including prioritized problem lists, SNAP, client-reported goals, measurable and achievable objectives, and interventions.
\u25cf Provide individual and group counseling sessions utilizing Evidence-Based Practices.
\u25cf Provide Family Group Counseling Services(if needed).
\u25cf Document client progress toward treatment plan goals and objectives in individual progress notes within state and federal standards and timeframes.
\u25cf Prepare effective care level step-down Transition notes.
\u25cf Develop solid discharge plans and summaries for the assigned caseload.
\u25cf Participate in treatment team meetings with other team members as needed.
\u25cf Participate in the agency’s Orientation training, staff meetings, and in-service training.
\u25cf Provide appropriate treatment interventions relative to the specific needs of each client.
\u25cf Adhere to the code of ethics, legal aspects of clinical practice, professional standards, duty to warn, abuse and neglect reporting policies and procedures, and exercise professionalism at all times.
\u25cf Exercise awareness in cultural competency, client satisfaction, quality of services, client needs, and overall health and safety of staff and clients.
\u25cf Completing EDD paperwork and submitting files
\u25cf Additional duties may be required if deemed necessary by the administration.
Required Knowledge, Skills, and Abilities:
1. Knowledge of SUD and Co-Occurring Disorders of industry standards as related to the position of BHT/Driver Support Staff, educated on the disease of addiction and substance abuse, co-occurring disorders, MAT, and Harm Reduction principles.
2. Ethical & Legal Considerations - Confidentiality and HIPAA regulations, Mandated Reporting, ethical decision making
3. Communication & Documentation - Clinical Documentation, Interdisciplinary Collaboration, Cultural Sensitivity & Trauma-Informed Care, Exit blocking
4. Basic knowledge and computer skills of PC software programs, particularly Microsoft Word and Excel, is preferred.
5. Communication skills - must be able to concentrate on work amid distractions such as noise, conversations, and foot traffic
6. Exercise Good Judgement - maintain self-control in volatile or hostile situations, such as when verbally or physically confronted in an aggressive manner
7. Work independently, with little supervision, in an unstructured environment with multiple demands
8. Basic Understanding of Treatment Program and Modalities - intake, assessment, biopsychosocial, drug screening, consents, record requirements and documentation, referral, and discharge planning

Group Facilitation Support, Treatment Planning Assistance, Supporting clients in following their treatment plans and setting recovery goals.
10. Cultural Sensitivity & Trauma-Informed Care - specific populations with deficits in care, harassment policies,s and cultural differences amongst co-workers and clients.
11. Interdisciplinary - Working with counselors, social workers, and medical staff
12. Positive Representation of Company Standards - Ability to represent the program well in interactions both inside and outside the facility
13. Community Education - knowledgeable about the process of recovery, 12 steps, and substance abuse treatment, and effectively educate those in the community
