Care Coordinator InternAt — Anaheim Fire & Rescue
The Care Coordinator Intern supports the Advanced Provider Response Unit (APRU) by assisting with patient care coordination, follow-up, and linkage to community-based services. This role provides hands-on exposure to mobile integrated health, behavioral health response, and interdisciplinary care delivery within a fire department–based program.
Tasks:
Assist with patient care coordination, including follow-up outreach and linkage to medical, behavioral health, substance use, housing, and social service resources
Support referral intake, tracking, and follow-through to promote continuity of care and reduced system utilization
Maintain accurate, timely, and confidential documentation in compliance with HIPAA and departmental policies
Collaborate with APRU clinicians, fire personnel, law enforcement partners, and community-based agencies to support coordinated service delivery
Participate in case discussions, program meetings, and care planning activities under supervision
Assist with discharge planning and coordination of next-step services
Support basic data collection, tracking, and reporting related to program outcomes and quality improvement
Review policies, procedures, and training materials to contextualize assigned tasks and understand program operations
Training:
Students placed with the APRU Care Coordinator Internship will receive a mandatory general orientation covering program overview, workplace expectations, confidentiality (HIPAA), safety, and documentation standards, followed by program-specific training on the APRU/Mobile Integrated Health model, care coordination workflows, and community referral pathways. Training will include supervised job shadowing and field exposure as appropriate, regular one-on-one mentorship meetings with an APRU supervisor, skills-based instruction using sample work products, and ongoing task contextualization supported by relevant policies and training materials. Continuous feedback will be provided to support learning objectives and professional development throughout the placement.
Learning Outcome:
Demonstrate foundational competence in care coordination by assisting with patient follow-up, referrals, and linkage to medical, behavioral health, and community-based services within a fire department–based Mobile Integrated Health (MIH) / APRU program.
Effectively communicate and collaborate with interdisciplinary partners, including APRU clinicians, fire personnel, law enforcement teams, and community agencies, to support coordinated and patient-centered care.
Apply principles of confidentiality, documentation, and compliance by accurately maintaining records and handling sensitive information in accordance with HIPAA, departmental policies, and risk management standards.
Identify and respond appropriately to the needs of vulnerable populations, including individuals experiencing behavioral health crises, substance use disorders, or social instability, while recognizing when to escalate concerns to clinical or supervisory staff.
Describe how data and outcomes support program evaluation and sustainability by assisting with basic tracking of referrals, follow-up outcomes, and service utilization within a public-sector healthcare program.
