Case Manager InternAt — SMA Healthcare

The Case Manager Intern will work alongside a SMA Healthcare Case Manager to provide resources and support to overcome any barriers to their recovery or wellbeing. 

Tasks:

-Shadow assigned case manger
-Complete their own comprehensive assessment, service plan and other required documentation
-Research resources in the service area and complete a guide with resources for all future case managers to utilize

Training:

The student will complete a 2 day general SMA Healthcare orientation. Following orientation, the student will orient to the program through training in the electronic medical record system, as well as job shadowing with an assigned case manager. The student will receive weekly one on one supervision with the site supervisor to evaluate progress and will be included in bimonthly peer review meetings.

Learning Outcome:

By the end of the academic internship, the student will be able to write and complete their own comprehensive assessment, service plan and document interactions with clients. The student will also understand the resources in the area and how to link individuals to services.
