The interim role ensures continuity of care, regulatory compliance, and effective psychosocial services during periods of transition, vacancy, census growth, or survey preparation.
Apply psychosocial assessment principles within a Skilled Nursing Facility setting Develop and update individualized care plans in collaboration with the interdisciplinary team Navigate discharge planning, resident rights, and grievance processes Communicate effectively with residents, families, and healthcare professionals Demonstrate understanding of CMS, state, and regulatory requirements related to social services Maintain accurate, compliant documentation and progress notes Respond appropriately to behavioral, emotional, and adjustment-related concerns Exhibit professional judgment, ethical practice, and adaptability in a transitional care environment
