Organization Description

SBHH Mission: Provide safe, trauma-informed, recovery-oriented inpatient psychiatric treatment that stabilizes acute symptoms, reduces risk, and supports discharge to the least restrictive level of care through integrated, interdisciplinary services. Program/Setting: Adult acute inpatient psychiatric hospital/unit providing 24/7 crisis stabilization, psychiatric evaluation, medication management, psychosocial assessment, safety planning, therapeutic group programming, and comprehensive discharge planning in collaboration with psychiatry, nursing, and case management. Population/Demographic: Adults (18+) experiencing acute psychiatric decompensation including mood disorders, psychotic disorders, severe anxiety disorders, and co-occurring substance use disorders. Many patients present with significant psychosocial stressors such as homelessness, limited support systems, legal involvement, and medical comorbidities. Admissions may occur on voluntary or involuntary status in accordance with state law.

Program Information

1. Conduct supervised psychosocial assessments, including psychiatric history, psychosocial stressors, strengths, support systems, and safety risk factors, contributing to individualized treatment plans. 2. Provide brief individual supportive counseling and skill-based interventions (e.g., grounding, distress tolerance, emotion regulation, coping strategies) appropriate to patient acuity. 3. Participate in safety planning and risk management interventions under supervision, including suicide risk assessment support and crisis stabilization interventions. 4. Assist with individualized discharge planning by coordinating outpatient services, housing resources, benefits linkage, and transportation planning. 5. Complete clinical documentation including progress notes, collateral contacts, and discharge-related documentation with attention to medical necessity and ethical standards. 6. Co-facilitate and independently facilitate therapeutic and psychoeducational groups such as coping skills, communication, boundaries, relapse prevention, and wellness planning. 7. Participate actively in interdisciplinary treatment team meetings by presenting psychosocial updates, identifying barriers to care, and collaborating on treatment and discharge goals. Conduct supervised collateral contacts and family support meetings providing psychoeducation and coordinating care with external support and providers.

Health & Safety

Because this is a psychiatric hospital, we work with individuals who may exhibit a wide range of behaviors. At times, situations can become intense. We also serve patients from the community who present with various health conditions that must be taken into consideration. However, our staff are highly trained to manage situations that escalate, and each patient undergoes a comprehensive evaluation so we are aware of any medical or behavioral factors that they may have.
